Genital Herpes: A $35.3 Billion Economic Burden on the Global Economy

Genital herpes carries a far larger economic cost than most people realize.

A study from researchers at the University of Utah Health and the World Health Organization estimated the global annual cost at $35.3 billion, driven primarily by herpes simplex virus type 2 (HSV-2).

That figure covers direct medical expenses as well as lost productivity, and it is almost certainly an undercount given how many people with herpes never seek treatment.

In 2016, HSV-2 accounted for approximately $31.2 billion of that total, with HSV-1 genital infections contributing around $4 billion.

Direct medical costs, including antiviral medications, clinical visits, and testing, came to $22 billion.

Lost productivity added another $12.3 billion.

Researchers noted that if every person with symptomatic genital herpes sought treatment, the economic burden could nearly double.

The Western Pacific region and the Americas bear the largest shares, at 34.6% and 24.4% of total costs respectively.

High- and middle-income countries account for roughly 76.6% of the overall financial impact, largely because higher rates of diagnosis and treatment drive up recorded costs.

In lower-income settings, most costs go unrecorded because most people never receive a formal diagnosis.

Beyond direct costs, the quality-of-life impact is substantial.

Recurrent outbreaks cause pain, embarrassment, and psychological distress.

People with HSV-2 also face a roughly threefold increased risk of contracting HIV, which adds to the downstream economic and public health burden.

In low- and middle-income countries, the HIV interaction alone has been estimated to account for nearly a third of antiretroviral medication costs and related wage losses.

There is currently no vaccine or cure for herpes. Researchers and public health experts increasingly argue that the economic case for developing one is strong. A vaccine that reduces transmission rates or outbreak frequency at scale could save billions annually in treatment and productivity costs, making it one of the more cost-effective investments available in infectious disease prevention.

Policy decisions around herpes vaccines will ultimately depend on cost-effectiveness analyses and public demand. A successful therapeutic vaccine, one that reduces outbreaks and transmission in people already infected, would reach a much larger market than a purely prophylactic one given how many hundreds of millions of people already carry the virus.

A study published in PLOS Medicine added further detail on low- and middle-income countries specifically, estimating that genital herpes contributed over $800 million in treatment and productivity losses in 2019 alone in those settings.

The HSV-HIV interaction was a major driver: the increased HIV susceptibility caused by HSV-2 infection generated costs that dwarfed the direct herpes treatment expenses.

The core message from this research is that herpes is not a trivial condition from a population health perspective. Its global economic footprint is larger than that of many diseases that receive far more attention and funding. Closing that gap in priority would likely require both better public awareness and a clear demonstration that vaccine investment can deliver measurable returns.
